1. Required athletic paperwork for returning student-athletes,
including insurance information, nutritional supplement information, etc. can be completed at the link below:
On-Line Student-Athlete Forms
Please have a copy of your insurance card ready to upload in the insurance section.  A picture of the card will also be sufficient. Â
These forms can be completed from a computer or tablet device.
2. In addition to the above athletic participation forms, all
student-athletes MUST click on the link below to fill out/ and or check to make sure that your General, Address, Emergency Insurance and Medical information is accurate under My Info (this must be completed from a computer. Tablets/smartphones do not currently work):

Directions for Sportsware:
1. Enter your Franklin College email address __________@franklincollege.edu (no
capital letters) and click the "Reset Password" button.
**Your FC email password IS NOT your
Sportsware password. You must click the "Reset Password" button to
have it sent to your FC email.
(If your email is not recognized, email Chris Shaff, Head Athletic
Trainer at cshaff@franklincollege.edu from
your FC email account)
2. Check your FC email account (mail.franklincollege.edu) and follow the link in
the email that you have received.
3. Return to Sportsware and log in using your newly created
password.
4. Complete each tab on the left hand side: "General",
"Address", "Emergency", "Insurance",
making sure to click SAVE once complete each time.
** THE ASTERISKS ARE REQUIRED TO COMPLETE THIS FORM **
- Be as thorough as possible and fill in as much information as
you can.
- If you are a returning athlete, please check your information to
make sure that it is current and update your "History".

**Returning student-athletes are NOT required to have a yearly
physical. Your initial physical completed your first year of competition is
sufficient for your four years of eligibility. Although, this physical is not
required, we certainly recommend a yearly physical. The link for the athletic
physical is below:

If you chose to have a new physical completed, pleast have it mailed in, the On-Line Student-Athlete Forms and Sportsware completed prior to August 4th.
