Freshman & Transfer Athletes Required Forms

1. All freshman and transfer student-athletes are required to have an Athletic Physical completed by a physician prior to their initial season of competition.  We cannot accept physicals that were completed PRIOR to June 1.  We also CAN NOT accept any other physical form than the Franklin College Athletics Physical Examination Form.  High School Athletic Physical forms are not acceptable.

Once completed, please mail into the address listed at the bottom of this page.

2.  Required athletic participation forms for freshman/transfer student-athletes,
including insurance information, nutritional supplement information, etc. can be completed at the link below:

On-Line Student-Athlete Forms

Please have a copy of your insurance card ready to upload in the insurance section.  A picture of the card will also be sufficient.  

These forms can be completed from a computer or tablet device.

3. All student-athletes MUST click on the link below to Join Sportsware and create a new account associated with Franklin College.

  1. Go to: www.swol123.net
  2. Click Join Sportsware button in the Athlete/Parent Block.
  3.  Enter in the School ID provided by your institution to access SWOL. (If you did not receive this unique code, contact your head coach or Chris Shaff, Head Athletic Trainer at cshaff@franklincollege.edu). Click Next.
  • ?Note: This is not your Student ID
  1. Next, fill in all your information. You MUST use your school email address (____._____@franklincollege.edu) or your Join request will be denied. 
  2. Confirm the information that you are submitting (Name, DOB, email) on the next screen and click Finish. 
  3. Your Join request will be sent to the Athletic Trainer for approval.  Once approved, you will receive an email with a link to continue.
  4. When your account is approved, you will be sent an email to create a password. Instructions for this step can be found here.
  • The link within this email is only valid for 24 hours.
  • If you miss the time frame, use the Reset Password link under Sign In to proceed.

Please have your completed physical mailed in, the On-Line Student-Athlete Forms and Sportsware completed prior to August 4th.
